Frequently Asked Questions

Where is the clinic located?

The clinic is in the St. Dominic West Medical Office Building, located on the grounds of St. Dominic Hospital. We are on the 14th floor, Suite 1460.


What do I need to bring to my appointment?

Please be sure to bring your insurance cardsdriver’s license, and a list of any medications that you currently take to every visit. Also, please bring any reports or x-ray films that you have had recently.


Do I need to pay at checkout?

Co-pays are due at the time of service. We will then file insurance claims for any insurance company with which we participate and bill you for any balance due for deductibles or difference on co-pays.


Do you accept credit/debit cards for payment?

We accept cash, checks, debit cards, MasterCard, VISA and Discover Cards. Payments can be made in person or by phone to our business office.


Do I need a referral to see one of the surgeons?

We do ask that all new patients be referred from a practicing physician or nurse practitioner; however, patients with an obvious surgical problem may be seen without a referring physician.


What are the office hours?

Our office hours are 7:30AM to 5:30PM, Monday and Tuesday and 8:00AM to 5:30PM Wednesday and Thursday.. On Fridays, we are open from 8:00AM to 1:00PM. Each doctor has his own scheduled days during the week to be in the clinic, as he performs surgery around his clinic times. We have a 24-hour answering service for after-hours calls. One of our physicians or nurses will always be available to return your call.


Where can I park?

Parking is available in front of the West Medical Tower, in a parking garage located to the right of the front parking lot, or in the parking garage under the tower. A security guard will direct you as to where to park. Free valet parking is also available for anyone who needs this service. Handicapped parking is available in the parking garage under the tower.


How do I get prescription refills?

Either call our clinic to request any refills needed or ask your pharmacist to call or fax our office with the request. We ask that refills and pain prescriptions be obtained during regular office hours.


Who notifies my insurance company of impending surgery?

Our clinic will assist you in pre-certification of your surgical procedure with your insurance company. The patient and/or policyholder will need to verify their benefits, that their procedure is performed in an “in-network” facility, and that all managed care restrictions are followed.


With which insurance companies do you participate?

Currently, we are participating with Medicare, Blue Cross-Blue Shield, Aetna, United Healthcare, TriCare, Humana and many others. Please check with our office and/or your insurance company to verify that benefits will be paid as an “in-network” provider. Our business office is available to assist you in this regard.


How is my surgery scheduled?

Once a decision to schedule surgery is made with your doctor, our scheduling nurse will assist you. She will make your reservations with the hospital, schedule with the operating room and anesthesia, and make any necessary appointments for preoperative lab work or consultations with other physicians involved. You will be given information that you will need to take to the surgery appointment. You will also receive information about any surgical preps that you will need.


What if I need to change or cancel my surgical reservation?

You will need to contact our scheduling department to change or cancel any reservations that have been made on your behalf. You may also contact them if you have any questions regarding the information given to you.


What do I need to do to obtain a copy of my medical records?

You will need to sign a medical records release form stating the dates of service that you need released, as well as the name and address of where the records should be sent or faxed. You will find these forms on our website under ‘Patient Forms.’


Do you complete disability claims?

Yes, our office is happy to complete your disability/cancer policy forms. Please complete the patient/policyholder information and forward them to our office. We will then complete the physician statement showing the disability dates and forward them to your insurance company or the patient.
